This form combines the Prenuptial Investigation Forms needed by the Tribunal with an optional Marriage Preparation Registration Form. It is a fillable PDF format that can be saved electronically or printed and filled out by hand.
Marriage Preparation Lead Couple Observation Form
This is an optional form that allows a marriage preparation lead or mentor couple to provide feedback to the witnessing clergy. It does not need to be included in paperwork sent to the Tribunal.

The Vatican has offered direction for the future of marriage preparation in the Church, moving us toward a model more closely resembling OCIA. While this will take time and careful thought to implement, it is worth taking time to get acquainted with this new model.
