Your intended deferred gift when using this form should be a gift bequest for your parish or a parish ministry or school. There is more than one field in case you are designating your gift to different funds at your parish. Perhaps the parish endowment or a capital improvement fund, or even it’s religious education program.

Your parish can help you learn the benefits of joining their parish legacy society.

Your parish and the archdiocese understands that estate plans can change so your enrollment in your parish legacy society does not bind your deferred gift in any way. The Trustee or Executor of your estate will notify your parish or archdiocese of your gift, directly.

Note: If your spouse has a separate estate plan document, please fill out a separate enrollment form for each.

Your enrollment in your parish’s legacy society may be shared with the pastor of that parish. We do not need the value of the gift unless you would like to share it or a percentage of your estate in the field provided. A note of your intended deferred gift will be recorded in your archdiocesan constituent record in the archdiocese’s Office of Stewardship and Development and is confidential and will not be published without your approval in any way.
